Project Goal:

This project focused on building an AI-enabled home care platform for senior health programs, helping a community-based care provider connect seniors and adults with trained Care Pros for nursing, therapy, and personal care; improving accessibility, coordination, and quality of life at home.

 

The Challenge

  • Seniors and families faced difficulty accessing timely, coordinated in-home care services.
  • Manual scheduling and matching processes slowed down care delivery and increased administrative burden.
  • Limited visibility into care visit status led to communication gaps among care recipients, families, and Care Pros.
  • Scaling personalized care across a growing population required a more efficient digital infrastructure.
  • Fragmented systems for booking, payment, and care plan management reduced operational efficiency.

Our Solution

  • Developed a mobile-first platform enabling users to request and manage care services anytime, anywhere, improving accessibility for seniors and caregivers alike.
  • Built an intelligent care-matching system connecting individuals with appropriately trained Care Pros based on medical and personal needs.
  • Integrated visit scheduling, management, and secure payment into a single streamlined experience, reducing administrative overhead.
  • Implemented real-time care request and coordination tools to minimize delays and improve service reliability.
  • Designed the system to support a wide range of services from personal care and nursing to therapy and home medical visits, within one unified platform.
  • Ensured the platform could scale to support growing demand across multiple regions and care categories.

This AI-enabled home care platform for senior health programs was built around a simple principle: technology should make compassionate care easier to access, not harder. By combining smart matching, streamlined logistics, and mobile accessibility, the solution empowers care organizations to serve more people with greater consistency and dignity.

How it Works (System Flow):

  1. A user downloads the mobile app and creates a profile outlining their care needs.
  2. The system generates a personalized care plan based on the individual's health and mobility requirements.
  3. An AI-driven matching process connects the user with a suitably trained Care Pro.
  4. The user books and schedules visits directly through the app.
  5. Care Pros receive visit details and deliver in-home services as scheduled.
  6. Users and families can track visit status and communicate through the platform.
  7. Payments are processed securely within the app upon completion of the visit.
  8. Visit history and care plans are stored to ensure continuity and support reporting.

Project Impact & Benefits:

  1. Reduced administrative coordination time by an estimated 40–50% through automated scheduling and matching.
  2. Improved care accessibility, enabling on-demand service requests across a wide range of medical and personal care needs.
  3. Supported delivery of over 1 million caregiving hours since platform inception, reflecting strong operational scalability.
